- The reappointment of the resident depends on the status of his/her quarterly performance evaluation.
- The performance evaluation report shall be discussed with the Resident (ratee) and the rater (Chairman). Should the rate be not satisfied with the grade given, he/she shall be given a prescribed period f 15 days from the date signed to submit letter of grievance addressed to the PERC thru channels. The Committee shall investigate the complaint and recommend to the MedicalCenter
- The following requirements aside from the performance evaluation report must be submitted to the Chief Education Training and Research Office one month prior to date of renewal – proposed or completed depending on the requirement of the department.
Penalty:
1st Semester Unsatisfactory Performance – residents is notified
2nd Semester Unsatisfactory Performance – warning given
3rd Semester Unsatisfactory Performance – non-renewal of residency.
Poor Performance in any quarter – non-renewal of residency.
Chief II.
- Rotation record (dates, area of assignment, specific skills acquired).
- Outstanding administrative accomplishment (performance rating, merits, special projects, awards).
- Training plan for incoming year level.
- Failure to submit requirements for renewal beyond date of appointment will mean dropping from the rolls. He/She will not be allowed to report to the hospital until he/she accomplishes the same requirement of a new employee.
Recommendation letter/forms properly signed for renewal by the Medical Credentials Committee must be submitted to the HR Office as scheduled.
